Volunteering

By consenting to this form, I am stating that the above information is true to the best of my knowledge. I understand the mission, vision and values of this organization. As such, I will conduct myself accordingly. I understand this is not an employment application and I will not be paid for my services. HSGC retains the right to terminate my volunteer involvement at anytime. HSGC is not liable for any injuries during volunteer time including transportation to and from the event. This consent will serve as my electronic signature for my minor child.
